What is a Social Prescriber?
A Social Prescriber provides an informative service to our patients and is here to help direct you to community group, activities and support services.
How can I get access to the Social Prescriber?
You as a patient can access this free service provided by the Social Prescriber by asking your GP, or surgery staff to refer you. You will then be contacted by the Chanel to come and and see her face to face or via telephone.
What services can I access through the Social Prescriber?
Our Social Prescriber can get you in touch with a Link Worker who can assist with a range of issues for example:
